An uncommon case of primary synovial sarcoma of the lung with a rare intraoperative finding

We report a case of 32-year-old male who was evaluated for right chest pain and cough with streaky hemoptysis. Chest X-ray contrast-enhanced computerized tomography scan the showed large (20x16×16 cm), heterogeneously enhancing mass arising from lower lobe. Ultrasound-guided Tru-cut biopsy revealed spindle cell sarcoma. In immunohistochemistry, tumor cells expressed epithelial membrane antigen, CD99, bcl-2. The final diagnosis primary pulmonary synovial sarcoma confirmed after positron emission tomography-computed that lesion confined to lung. patient managed by multimodal treatment. underwent anterolateral thoracotomy lobectomy staged systematic mediastinal lymphadenectomy. Intra-operatively, he given 2 units packed red blood (RBCs). developed dark-colored urine near end transfusion second unit RBCs, but without any hemodynamic instability. All tests possible reactions were negative; thus, we concluded this event probably secondary lysis due handling lesions during surgery.
